Complete Your Immunization Requirements
Students pursuing careers in healthcare must be adequately immunized in order to reduce the risk to themselves as well as the person under their care.
The immunization requirements at Bryan College of Health Sciences are based upon guidelines for students in health sciences and for healthcare workers in general. All students are required to comply with the College's immunization policy and submit documentation of immunizations prior to enrollment.
Complete the student health forms as soon as possible. You may need to contact your healthcare provider to obtain this information. All required immunizations must be complete in order to submit the form. If you do not have all required immunizations, you will need to contact a health facility that disperses the immunization in order to complete all requirements. Once your forms with all required immunizations are submitted, you will need to upload your immunization record(s) in your Student Portal under My Documents. Student Health will then verify your forms with your supporting documents.
- Check your state immunization portal for your immunization records, print the PDF and upload it to your Student Portal. Nebraska's can be found here.
- The Student Portal will only allow you to upload file type PDF; you may need to convert your file into PDF format in order to upload the required documentation
- If you need help on converting your file, we have provided a few options here
- Here is a tutorial on how to upload documents in the Student Portal
The login credentials for the student health forms are the same as the enrollment and enrollment deposit forms.
